Routing Plan Enforcement — Full Governance Lifecycle
How a governance lifecycle handles defects, behavioral incidents, and remediation loops in production.
This case study traces a single workstream from specification through final approval, documenting every phase transition, every review finding, and one behavioral incident that occurred mid-execution. The workstream under examination is notable for two reasons:
- It is self-referential. The work being governed was the construction of the system designed to detect and prevent agents from violating their own routing plans. During its own construction, an agent violated the execution path — and the governance framework caught it.
- It is complete. The lifecycle traversed every phase of the governance model, including two remediation loops triggered by real defects found by independent review agents, and an incident during execution that was detected, documented, and correctly diagnosed.
The case study includes the machine-generated audit trail with entry IDs, timestamps, author roles, actions, and human approval gaps recorded as they occurred. Two critical defects were caught before any code reached production. One behavioral incident — an agent reading work instructions from memory rather than the governance control plane — was detected, classified, and added to the incident corpus.
