Original content published on agentgovernance.org — including incident records, pattern descriptions, articles, and editorial commentary — is published under the Creative Commons Attribution 4.0 International License (CC BY 4.0) unless otherwise noted.

You are free to share and adapt the content for any purpose, including commercial use, provided you give appropriate attribution to AI Agent Governance (agentgovernance.org).

Accuracy and disclaimer

Incident records and pattern classifications are based on available information at the time of publication. We make reasonable efforts to ensure accuracy but do not guarantee that all information is complete, current, or error-free.

Content on this site is provided "as is" without warranty of any kind, express or implied. AI Agent Governance is not liable for any decisions, actions, or outcomes based on information published on this site.

Illustrative cases

Some incident records are marked as illustrative cases. These are constructed examples based on observed patterns in production AI agent systems. They represent realistic failure modes but do not describe specific real-world events. Illustrative cases are clearly labeled on the site.

User submissions

Users may submit incidents through the site. By submitting an incident, you represent that the information provided is accurate to the best of your knowledge. Submitted incidents are subject to editorial review and may be edited, classified, or declined at our discretion.

Do not submit confidential, proprietary, or personally identifiable information through the incident submission form. We are not responsible for protecting information voluntarily submitted through public forms.

Newsletter

By subscribing to the AI Agent Governance newsletter, you consent to receiving periodic emails about new incidents, pattern updates, and governance-related content. You can unsubscribe at any time using the link in any newsletter email. Newsletter delivery is handled by Beehiiv; their terms apply to the email service.

Prohibited use

You agree not to:

• Use the site or its content to harass, defame, or misrepresent individuals or organizations
• Attempt to interfere with the operation of the site or its infrastructure
• Misrepresent affiliation with AI Agent Governance

Automated access and data collection

You may not use automated tools, scripts, bots, or any programmatic means to access, scrape, crawl, harvest, or download content from agentgovernance.org without prior written permission, except as follows:

• Search engine crawlers that comply with our robots.txt directives are permitted
• The RSS feed (/rss.xml) is available for standard feed reader consumption
• Individual pages may be accessed at a reasonable rate for personal, non-commercial research

Prohibited automated activities include, but are not limited to:

• Bulk downloading or systematic retrieval of incident records, pattern data, or article content
• Building derivative databases or datasets from site content without attribution and permission
• Circumventing rate limits, access controls, or other technical measures
• Using automated tools to submit forms, create accounts, or interact with site features
• Training machine learning models on site content without written authorization

We enforce rate limiting and automated access detection. Violations may result in immediate and permanent IP blocking without notice. We reserve the right to pursue legal remedies for unauthorized automated access, including claims for damages and injunctive relief.
